Building Permit Applications

Below you will find links to downloadable forms needed for your project. Please be advised that before beginning any project or filling out an application, you may wish to contact Building Inspector, Dennis J. Butteris, for more specific information.

Plan Commission Forms

Please use these forms to appear before the Town of Baraboo Plan commision. Plan commission meetings are normally on the first monday of the month at 6:15 PM at the Town Hall. Forms and fees are due at the town by the second monday of the month in order to appear on the next plan commission meeting.
Fees start at $275

LIQUOR AND OPERATORS’ LICENSES
Please visit the Town Clerk at 101 Cedar Street to fill out the appropriate application. Those applying for an operator’s license, often called a bartender’s license, will need to present a valid photo ID and be aware that a background check will be run.
The fee for a new operator’s license applicant is $25.

DOG LICENSES
In order to obtain a dog license, please complete the form below and deliver in-person or mail to the Town of Baraboo Treasurer, Mary Farrell-Stieve. Note that no dog licenses will be issued in the month of December.

The cost is $5 per license for spayed or neutered dogs and $10 for those that are intact.
